Employees can contribute from 2% to 75% of eligible pay on a pre-tax and/or Roth 401(k) after-tax basis, up to the IRS limits. If you do not enroll yourself or opt out, you will be automatically enrolled in the plan within 30 days of hire at a contribution rate of 3%, on a pre-tax basis.  

 • Additional catch-up contributions are available if you are age 50 or older.  

 • After you attain one year of service, Medline will match $0.50 for every $1 you contribute.  

 • Medline’s maximum annual company contribution is up to 4% of eligible pay, meaning you must contribute 8% or more to receive the max.  

 • If you stay at a 3% contribution, it will automatically increase by 1% after one year, and annually thereafter, up to 12%.  

 • All active employees are 100% vested in both their contributions and the company match.

Retirement Savings and Planning 401 (k)

Regular full time employees (excluding Commissioned Sales Representatives) are eligible to receive: 

Up to 10 days in first year of employment, based on hire date.  After two years of employment an additional day is added in the following year and each year thereafter, up to a maximum of 19 days of paid vacation is earned at 10 years of employment.

Maternity & Paternity Leave: 

Coverage begins after one year of employment. 

Medline provides up to six weeks of paid parental leave to benefit eligible employees following the birth of an employee’s child or the placement of a child under the age of 18 in connection with adoption, a court-approved guardianship or legal foster care. Birth mothers may also qualify for short-term disability (STD).

Child Care

There are three basic PPO type plans through UnitedHealthcare and an HMO plan with Kaiser Permanente (KP) for California and Hawaii employees:  

 • UHC Gold Choice Plus PPO: A traditional PPO plan 

 • UHC Saver HSA: A high deductible plan with health savings account option  

• UHC White HRA: An in-network only plan with health reimbursement account  

• KP HMO*: California and Hawaii in-network only HMO plan *Available to employees scheduled to work in Hawaii at least 20 hours per week. 

Health Insurance

Medline will fund $750 for employee-only coverage and $1,500 for employee +1 or family coverage throughout the year to employees enrolled in the Saver HSA Plan. Employees may also contribute on a pre-tax basis. 

Health Savings Account (HSA)

Medline automatically provides basic Group Term Life Insurance. 

Medline automatically provides up to 1.5x times your annual base pay or $150,000 for commissioned sales representatives.

Life Insurance

You may purchase additional life insurance and/or accidental death and dismemberment in $10,000 increments, up to a maximum of $500,000.

The cost is determined by age and the amount of coverage. Guarantee issue limits may apply.

Life and Accidental Death and Dismemberment (AD&D) Insurance

Short-Term Disability (STD) (after one year of employment)—After one week of absence due to a non-work-related illness or injury, STD provides 60%, capped at $2,500/week of eligible pay for a maximum of 25 weeks. Pregnancy STD provides 100% for up to seven weeks (no cap) and 80% thereafter.  
 
Long-Term Disability (LTD) (after one year of employment)—For disabilities lasting more than 26 consecutive weeks, benefits are paid at 60% of eligible pay, to a maximum of $10,000 per month.

Disability

Eligibility for undergraduate and high school diploma course reimbursement begins after 30 days of employment for full-time employees working 40 hours/week. Eligibility for graduate school reimbursement begins after one year of employment. Tuition is reimbursed at 80%; books at 50%.
